Preparation Requirements
Obtain the Medical DocumentÂ
Request the medical letter, health certificate, vaccination record, or other medical document from the healthcare provider or issuing facility. Â
Verify the DocumentÂ
Ensure the document contains the patient's relevant information, the provider's name, and any necessary signature or certification. Â
Arrange NotarizationÂ
For privately issued medical documents, an authorized signer may need to sign an appropriate statement before a commissioned Florida notary public. Â
Check Official RecordsÂ
Government-issued medical or health records may require official certification rather than notarization. Requirements depend on the issuing agency. Â
Submit for ApostilleÂ
Submit the eligible original document with its required original certification or notarization to the Florida Department of State. Â
Important RestrictionsÂ
Florida notaries cannot certify copies of public records when official certified copies are available. Confirm any translation or additional requirements with the receiving country.
Required Certification
Privately Issued Medical Documents
Medical letters, health statements, vaccination certificates, and other privately issued healthcare documents may qualify for a Florida apostille when an authorized signer executes an appropriate document with a valid Florida notarization.
Florida Notarization
The notarial certificate must be in English and include the appropriate notarial act, date, venue, signer's name, required identification information, notary's original signature, and official seal.
Government-Issued Medical Records
Medical or health records issued by Florida government agencies may require an official certified record bearing an eligible government signature.
Notarization Restrictions
Florida notaries cannot certify photocopies of public records when certified copies are available from the government record custodian.
Original Document Requirement
Submit the eligible original certified document or original properly notarized document. Ordinary photocopies are not accepted.
Important Note
Acceptance depends on the document's issuing authority and certification. Confirm specific preparation requirements before submitting medical or health records for apostille processing.
