Preparation Requirements

Identify the Adoption Document

Determine whether the document is a final adoption judgment, adoption decree, amended birth certificate, adoption-related court order, or another adoption record.

Obtain Certified Court Documents

For adoption judgments, decrees, and other court records, contact the Clerk of Court where the adoption was finalized to determine how an authorized person can obtain an official certified copy.

Confidential Adoption Records

Florida adoption proceedings and records are generally confidential. Access to sealed adoption records may require a court order or other legal authorization.

Amended Birth Certificates

If an amended Florida birth certificate was issued following adoption, request an official certified copy from the Florida Bureau of Vital Statistics or an authorized issuing office.

Ensure the document contains the appropriate official certification and an eligible government official's signature.

Confirm Destination Requirements

Ask the receiving country or institution whether it requires the adoption judgment, amended birth certificate, or other supporting documents.

Submit for Apostille

Submit the eligible original certified document to the Florida Department of State.

Important Restrictions

Ordinary photocopies, uncertified court printouts, and privately notarized copies of public records are not acceptable substitutes for official certified records.

Access to confidential adoption records must comply with Florida law.

Required Certification

Adoption Judgments and Decrees

Florida adoption judgments, decrees, and related court orders generally require an official certified copy issued by the appropriate Florida Clerk of Court.

Court Clerk Certification

The certified court document must contain an eligible official certification and signature that the Florida Department of State can authenticate.

Confidential and Sealed Records

Florida adoption court records are generally confidential. A court order or other applicable legal authorization may be required before certified records can be released.

Amended Florida Birth Certificates

An amended birth certificate issued after adoption must be obtained as an official certified record bearing the State Registrar's signature.

Notarization Restrictions

Florida notaries cannot certify photocopies of adoption court records or other public records when official certified copies are required.

Original Certified Document

Submit the eligible original certified court record or birth certificate. Ordinary photocopies are not acceptable.

Important Note

An apostille authenticates the eligible government official's signature and authority. It does not determine parental rights, adoption validity, or whether the document will satisfy the receiving country's legal requirements.
