Preparation Requirements
Identify the Medical or Health Document
Common documents include physician letters, medical certificates, vaccination records, medical examination reports, health statements, and certified copies of patient records.
Obtain the Document From the Issuing Provider
Request the required document from the Alabama physician, hospital, clinic, or healthcare provider that issued or maintains the record.
Physician Letters and Medical Certificates
Ask the healthcare provider to prepare the letter or certificate on official letterhead, including the provider's name, professional credentials, and signature.
Confirm whether the receiving country requires a particular medical form, specific examination information, or additional professional verification.
Medical Records and Examination Reports
Request a complete and legible copy of the required medical record or report.
If the document requires apostille through notarization, ask the healthcare provider whether an authorized representative can sign an appropriate certification statement before an Alabama notary.
Vaccination and Immunization Records
Obtain the record from the healthcare provider or responsible issuing authority.
Electronic printouts or unsigned records may require additional certification or notarization before they are suitable for apostille processing.
Alabama Notarization
Privately issued medical documents generally need a properly notarized signature or accompanying certification to use Alabama's notarial apostille process.
The notarization must be performed by a currently commissioned Alabama notary public and contain the notary's original signature and official seal.
Government-Issued Health Documents
Official documents issued by government health authorities may follow a different certification procedure. Their eligibility depends on the issuing official's signature and whether Alabama can authenticate it.
Foreign-Language Documents
Documents written in a foreign language must be accompanied by a notarized English translation when presented for Alabama authentication.
Important Information
Medical records contain sensitive personal information. Only provide the documents necessary for the intended international purpose.
National Apostille Center can review the document type and its certification before processing to help determine the appropriate preparation requirements.
Required Certification
Healthcare Provider Certification
Medical letters, reports, and privately issued health documents should be signed by the issuing physician or an authorized healthcare representative when certification of the document is required.
Alabama Notary Public
For the Alabama notarial apostille route, the document must bear the original signature and official embossed or ink-stamped seal of a currently commissioned Alabama notary public.
Proper Notarial Certificate
The notarization must identify the appropriate notarial act and contain a complete notarial certificate.
The notary verifies the signer's identity or administers the applicable oath or affirmation. The notary does not independently certify the medical information.
Government-Issued Records
Documents issued by Alabama government health agencies must be reviewed to determine whether they carry the signature and seal of an official eligible for direct apostille authentication.
A government agency's letterhead or printed seal alone does not automatically establish eligibility.
County Authentication
Additional county authentication of an Alabama notary's commission is generally not required when requesting an apostille.
For notarized documents requiring certification for a non-Hague country, Alabama requires additional county authentication of the notary's commission.
Apostille Issuing Authority
Alabama Secretary of State — Authentication Division.
Important Notice
An apostille authenticates the relevant public official's signature, seal, and capacity. It does not verify a medical diagnosis, confirm the accuracy of patient records, or establish whether a medical professional's qualifications are recognized abroad.
